Filling out these forms ahead of time will help save time when you come for your appointment.
Appointments
Patients are seen in the office by appointment only. Please call (559) 625-625-8674 (Visalia) or (559) 793-4400 (Porterville) to make an appointment. For your initial visit we request you to download and fill out the forms in the Forms section. If this is not possible please arrive in the office fifteen minutes early to fill out necessary paperwork.
Cancellation/ Rescheduling Policy
In order to be respectful of the medical needs our local community and valuable staff time, we ask that you call 48 hours before your scheduled appointment to cancel/reschedule. This will enable us to better utilize available appointments for our other patients in need of medical care.
